A contract needs to be completed for each student. After completing the first section of this form you'll be taken to section two where you'll have access to recordings of our K-5 and 6-12 orientation. Please view the video that pertains to your student
and answer the key questions to show your understanding of the responsibilities of the Course Based Instruction model at Academy of Innovation - you may need to view both recordings depending on the grade level of your student(s).
When inputting your child’s birthday below please use the mm/dd/yyyy format - for example 04/05/2010. Once all the sections of this form are successfully completed you'll be emailed the contract to the email address you provided in the form. Read the contract carefully and sign with the date of the live zoom orientation (August 9, 2022) if you attended or with the date on which you watched the recording(s).

Contract(s) can be turned in via email at ai@cnusd.k12.ca.us or in person at our Citrus Hills Intermediate School at 3211 South Main Street in Corona or JFK Middle College High School 1951Third Street in Norco.
